Forms for Customs and Border Protection

Once you are onboard the plane, you will have to fill in two forms issued by the Department of Homeland Security, Customs and Border Protection of the United States of America. One is the called the I-94W form (each passenger has to fill one in) and the other one is the 1651-0009 (only one is necessary per family.)

Filling them in is easy, you should not need help. However if you do need help, the cabin crew will help you with any queries you may have. To make things easy for you we have filled in the two forms. These are just examples and are not valid; the official forms will be distributed on the plane. You will also have more forms in Customs if you need to redo them. Do not hesitate to consult either the cabin crew or the Custom Officials with any doubts or queries you may have.
